Transition 101 for Special Education Teachers and Case Managers






Special education teachers and case managers often build the essential relationships with students and their families that make real outcomes happen for students with disabilities. Understanding what Transition Services are and how they help students realize dreams is essential to the transition process. This session combines three new trainings from the Student-Centered Transition Network to help secondary teachers and case managers better understand how to meaningfully contribute to transition services and transition assessment. Get the highlights on transition assessments, the writing of appropriate measurable postsecondary goals, as well as building and implementing the coordinated set of activities. Legal requirements for transition and best practices will be sprinkled throughout. Be empowered to help your students with long-term planning.
