Transition: Person-Centered Planning






This session integrates the tenets of PATH (Planning Alternative Tomorrows with Hope) with futures planning.  Participants develop customized protocols to guide students in short-term and long-term goal setting in multiple areas of postsecondary interests.  Participants will utilize a modified PATH process, and learn how to combine community and district supports with the student's circle of supporters.  Participants will receive a copy of PATH and MAPS Handbook, Hints for Graphic Facilitators, as well as a set of markers for illustration.
